How To Become a Patient at the Neighborhood Christian Clinic
Overview of Services
The Clinic is able to provide Medical & Dental services at an extremely low cost to patients because of donations and the support of volunteer healthcare professionals.
First time patients seeking medical care must participate in a random drawing. The number of patients drawn is based on the number of providers available – no visit is guaranteed. Patients not drawn are advised to come back at another time/day or to seek care at another clinic. Information for other low cost clinics is available from reception staff.
The dental clinic is currently closed to new patients.
Eligibility
Anyone who does not have health insurance nor qualifies for AHCCCS is eligible for treatment at the Clinic

Medical Fees
- Patient Visit $40
- Telehealth Visit $30
- Nurse Visit $10
- Nurse Visit w/ Meds $20
- Labs $40
- Medication Refill $20 admin fee
- Syringes / Strips $10 per box
- Lancets $5 per box
- Meter $10
- Well Women Visit $80 includes lab fees
- Ultrasound $40
- Eye Exam $20
NOTE:
• Patient will pay lab fees above $100
• Lab fees must be paid prior to labs being ordered
• X-Rays / diagnostic charges must be paid at time of
service
• If patient fails to keep an appointment, all future visits
must be paid for when appointment is scheduled

- Comprehensive or Limited Exam $50
- Dental Hygiene Visit $40
- Scale/Root Planing $100
- Restorations (fillings) $50 each appt
- Root Canal (Endodontics) $100
- Crowns $300
- Bridges $300 per abutment + $200
per missing tooth - Complete / Partial Metal Denture $400 / arch
- Complete / Partial Valplast Denture $300 / arch
- Interim-partial $100 + $10 / tooth $40 for wires
- Denture Reline $130 each
- Night Guard $150
- Immediate Dentures $500
NOTE:
- X-Rays, if necessary, are included in all dental visit fees
- Fees must be paid before services are rendered
- Cost of Dental Labs must be paid prior to labs being ordered
- Cost of hygiene visits must be paid when appointment is scheduled
